Beginning during the 2004 Fall Semester, Oklahoma State Law (70-1210-193) requires that all students who attend Oklahoma colleges and university in-person must provide proof of immunization for certain diseases. Read more here: Oklahoma State Vaccine Requirements (nvic.org)

In order to meet the requirements of this law all on-campus students must complete the linked Immunization Record [PDF] document before they move onto campus or participate in any curricular, co-curricular or extracurricular activities.
Medical, religious, moral, and philosophical exemptions are allowed by law and the details and required paperwork can be found in the linked Certificate of Exemption document.
